Dynamic / T-Mobile Dealership Application filled out (attached below) 2. Certificate of Resale / CRT61 filled out (attached below) 3. W-9 Form filled out (attached below) 4. Copy of SS-4 Form (proof of TIN [taxpayer identification number]) 5. Copy of your Driver’s License 6. 1 picture of your Store • 1 Exterior – capture of the entire store front. Top to bottom and left to right of the entire Store front. Please e-mail the completed application and pictures to Alejandra@adcompcs.com If you are unable to email the application, please fax it to 773-427-4909 Rules and Regulations 1. Must uphold T-Mobile brand image and are in traffic driving locations. 2. Should not show immoral business themes. (pornographic images and etc) 3. Should not market, promote and sell immoral retail goods. 4. Approved dealers will NOT be allowed to sell or activate Postpaid. As an Authorized Dynamic / T-Mobile Prepay Agents, you are committed to activate and sell PREPAY products and services ONLY. 5. No posters, clings and flyers shall be displayed on windows.
